COVID-19 Guidelines
My household members have not had any of the following symptoms in the last 21 days: sore throat, cough, chills, body aches for unknown reasons, shortness of breath for unknown reasons, loss of smell, loss of taste, fever at or greater than 100.4 degrees Fahrenheit.
My household members have not tested positive for COVID-19.
My household members have not visited or received treatment in a hospital, nursing home, long-term care, or other health care facility in the past 30 days.
My household members have not traveled in the U.S. or International in the past 21 days. (if yes, we have been quarantined for 14 days).
My household members are not health care providers or emergency responders. (If yes, please use your judgement).
My household members have not cared for an individual who is in quarantine or has tested positive for COVID-19.
No household members have been exposed or acquired COVID-19.
No household members have knowingly been in close proximity to any individual who tested positive for COVID-19.
In order for the Social Center to remain open we need your cooperation in respecting the above guidelines.
